New Smart Card
The following documents must be uploaded when applying online for a new family card.
- Photo of the head of the family member
- Aadhar cards of the head of the family and family members and birth certificate for children below 5 years of age in case of non-availability of Aadhar
- Any one of the following location sources
- Aadhar card
- Electricity bill receipt
- Front page of bank account book
- Gas Consumer Card
- Property tax receipt if own house
- Passport
- Lease Deed (for renters)
- Housing Allocation Order of the State Slum Clearance Board
- Telephone bill receipt
- Voter ID card
The applicant’s family is eligible to receive a new electronic family card after Field verification
Deletion of Family member Documents to be uploaded
- Aadhar card of the person whose name is to be deleted
- Documents stating the reason for removal of the name and supporting documents
- Proof of new residence in case the person to be deported resides as a separate family
- Death certificate should be uploaded in case of name removal due to death of family head or member.
- In case of deletion of the name of the married member, marriage certificate should be uploaded.
- Proof of new residence is sufficient if either spouse is in possession.
Duplicate Card
You can get a duplicate card to replace the lost and non-scanned cards in the POS machine. Apply online and pay the duplicate card fee of Rs. 20 / – at the Taluk Office and get the receipt.
